Accident & Sickness Hospitalization

Provides additional money to help offset expenses or the loss of income should a covered employee be hospitalized

• Provides hospitalization benefits for covered illnesses and accidents
• Benefits are payable for:
  • Hospital Admission - provides lump sum benefit payment of up to $400 when admitted to a hospital as an in-patient for medically necessary treatment
  • Hospital Confinement - pays up to $200 per day for each day of in-patient confinement
  • Intensive Care Rider - pays up to $1,000 for each day confined in an intensive care unit in a hospital
  • Outpatient Surgery Rider - pays $500 per accident or sickness for major outpatient surgery or $50 per accident or sickness for minor outpatient surgery. Only one benefit amount is payable per accident or sickness
  • Emergency Room Rider - pays $50 per accident or sickness for treatment in an emergency room (maximum of 4 visits per year)
• Spouse & children’s riders are available
• Guaranteed renewable until age 75 at which time the policy terminates
• Benefits are paid directly to the insured
• Benefits are portable - the insured can keep the coverage even if there is a job change

Provides additional money needed when an accidental loss of life or dismemberment occurs

• Accidental death benefit amount up to $500,000
• Dismemberment benefit:
  • 50% of the select death benefit
  • Loss of each limb
  • Maximum 2 limbs per accident
• Monthly settlement options available
• Spouse and children’s are rider available
• Non-cancelable and guaranteed renewable up to age 70
• Benefits are portable - the insured can keep the coverage even if there is a job change

Provides additional money needed when a covered accident and a resulting disability disrupts your life

• Total disability benefit from $250 to $2,000 per month, options of up to 6 or 12 months. Benefits begin with the first day of disability unless 7 or 14 day elimination period is selected
• Benefits are portable - the insured can keep the coverage even if there is a job change

Accident Hospital Indemnity Rider

• Daily hospital income benefit of $200 per day beginning with the first day of hospitalization
• Up to $ 1,000 per ride for air transportation and up to $200 per ride for ground transportation due to a covered accident (two rides per accident)
• $100 per covered accident if the insured requires treatment in an emergency room

Accident Treatment Rider

• Physician treatment benefit of $50 per covered acciden
• $50 per visit by a licensed physical therapist for up to 10 visits per covered accident when prescribed by a physician

Catastrophic Accident Rider

• Catastrophic benefits of up to $75,000 of coverage for accidental death or for loss of sight, speech, hearing or for loss of the use of limbs as the result of a covered accident

Family Rider

• Family hospital indemnity rider for accident and catastrophic treatment, the benefits pay the insured’s family half the amounts the insured purchased for him or herself
